- Passport size Photo,
- Proof of Identity (Driving License/Passport/Voter ID/Aadhar Card etc.),
- Certificates of your Educational Qualification,
- AMPI Chapter Name and Membership Reg# or Ref# if you are a member of our local AMPI chapters
